Transition Program
The goal of the Transition Program is to provide individuals and families exiting emergency shelters with assistance that successfully reintegrates the household into decent, affordable housing. Residents of the shelters are made aware of this program via the case management services they are receiving as a part of their shelter stay. Case managers provide relocation assistance, case management, mediation services, linkage to other social service and health care programs, and tenant education.
The Transition Program is administered by Community Shelter Board’s (CSB) Direct Client Assistance Administrator. This staff person coordinates with all of CSB’s partner agency shelters and others to ensure community wide coordination and access to the program.

There were 318 households served in the first six months of FY 2005 through the Transition Program and 100% of those served achieved successful housing outcomes. A successful housing outcome is defined as housing being stabilized and not re-entering the shelter system.
