Supportive Housing
Permanent supportive housing ends homelessness for chronically homeless individuals and families coping with mental illness, addiction or chronic illness. Supportive housing is permanent rental housing linked to a range of support services designed to help individuals with mental and/or physical disabilities maintain their housing and live fuller lives. By providing an alternative to expensive emergency public services, hospitals, jails, shelters, supportive housing is cost-effective and improves lives by enabling residents to live independently.

Results
For Fiscal Year 2004-05, a total of 665 distinct households were served through the Rebuilding Lives Supportive Housing Programs. Of this total, 219 households entered the program between July 1, 2004 and June 30, 2005. Another 446 households were already in the program prior to July 1, 2004.
Figure 1 below reveals the composition of total outcomes in terms of percentages. A successful outcome is either remaining in supportive housing or exiting to other permanent housing.
