Program Revision

If your agency is requesting a significant change in scope of services, please respond to the following questions on no more than four typed pages. Please note that programs may only request increased CSB funding via the competitive 7/1 grant cycle. Annual Shelter Funding is based on historical levels only.

Explain why the change is proposed.

Explain how the agency’s programs will change. Please answer the following:

What program changes or additions are proposed?

What is the schedule for implementation?

How will client services be affected?

What impact will proposed changes have on CSB’s desired system outcomes? More specifically, will the changes proposed reduce the average cumulative length of stay, improve cost-effectiveness, and reduce the need for shelter?

Will program changes require the adoption of a new service model? If so, please describe the model and its value. Indicate whether the model has been employed in other areas, including the names of other shelters or areas where it has been used and its impact.

Will program changes improve collaboration with other service providers? If so, please explain how.

Will the changes increase the agency’s capacity to provide services to difficult to serve populations? If so, please describe.

How will the sheltering system be affected? How will the agency be affected? Please include assumptions to support your rationale.

Describe impact, if any, on program budget? If additional revenues are required, what is the planned source(s) and likelihood of receiving?
