Program Narrative – Competitive Application

Please prepare a narrative response to the following questions. The narrative must be limited to six (6) pages or less. Please number and use CSB’s titles for each section of the narrative. Also provide a half page executive summary of the program narrative.

  1. Prior Experience:
  1. Explain your agency’s history in providing similar services.
  2. Indicate management strengths, technical and/or clinical strengths and existing community partnerships.
  3. Describe successful implementation of HRC-related services in the past (if applicable).
  1. Persons to be Served:
  1. Describe targeted population to be served by the project and how outreach will occur.
  2. Describe how the program will address cultural relevance to ensure active participation and access, as well as how a safe, respectful environment will be created.
  3. Describe how consumers will be connected to housing and employment.
  1. Statement of Guiding Values and Principles:
  1. Describe the core values, beliefs and principles that will drive the programming and activities and how these values fit into the overall mission of the applicant organization.
  2. Describe how consumers will know of the guiding principles.
  3. Describe agency’s knowledge, experience and commitment to providing services to persons who are homeless or at risk of becoming homeless.
  1. Consumer Focus: Describe how consumers will be incorporated into the program design and implementation.
  2. Program Description:
  1. Indicate which initiative for which application is being made (Homeless Prevention, Shelter Diversion, PSH for Women, RL Resource Center or RL HRS).
  2. Give a summary description of how the applicant organization will implement the proposed project.
  3. Describe specific project activities
  4. Outline timetables for implementation
  5. Describe community resources that will be utilized to achieve successful outcomes on behalf of the consumer.
  6. Define services and how they will be provided.
  7. Describe admission criteria
  8. Describe staffing patterns
  9. Describe termination procedures.
  10. Describe service design specifications.
  1. Administrative Capacity: Describe how the applicant will manage and administer the program. Address the following:
  1. Program evaluation
  2. Fiscal management
  3. Contract compliance.
  4. List key administrative staff and their qualifications.
  5. Attach a table of organization.
  1. Neighborhood Partnership:
  1. Describe how the applicant organization will partner with the neighborhood to successfully operate program.
  2. Describe previous experience in this area.
  1. Implementation:
  1. Will this project be ready for implementation July 1, 2001? If not, when will program be ready for implementation?
  2. Describe objectives related to start-up activity.
  3. If project is already operational, describe how receipt of CSB funding would affect activities and implementation.
